What you will need

Use the papers in this order.

  1. Before you start, have these ready.

    Have ready:

    • The child’s birth details and the names of the birth parents or guardian
    • ID and address proof of the adopting parents

  2. Make the adoption deed, signed by the adopting parents and the person giving the child, before two witnesses.

  3. If the child’s name is to change, a parent swears the affidavit for the change of the minor’s name.

  4. Fill in the municipal form to report the birth of an adopted child (Form 1-A), so the record carries the adoptive parents’ names.
