What you will need
Use the papers in this order.
Before you start, have these ready.
Have ready:
- The hospital’s birth slip or record, and both parents’ ID proof
- The date and place of birth, and the name chosen for the child, if any
Fill in the birth report (Form No. 1) and give it to the registrar of births and deaths within 21 days of the birth.
If the child was named after the birth was recorded, use the form to add the name to the birth record.
Apply for copies of the birth certificate using the application form.
Only if the birth was not registered in time: swear the affidavit for delayed birth registration and file it with the application.
