What you will need

Use the papers in this order.

  1. Before you start, collect these papers.

    Have ready:

    • Whatever you have of the lost or damaged letter, such as a photocopy or the allotment number
    • ID proof, Aadhaar card and PAN card of the applicant, and two witnesses for the indemnity bond

  2. Fill in the application for a duplicate allotment or transfer letter, possession slip or no due certificate.

  3. If a GPA/SPA holder is applying, fill in that holder’s affidavit.

  4. Fill in the applicant’s affidavit and the indemnity bond, and sign them as the forms ask.
