What you will need

Before you start, have these ready.
Have ready:
- The hospital’s or doctor’s record of the death, and the deceased’s ID proof
- The date and place of death, and your ID proof as the informant
Fill in the death report (Form No. 2) and give it to the registrar of births and deaths within 21 days of the death.
Apply for copies of the death certificate using the application form. Banks, insurers and offices usually ask for several.
Only if the death was not registered in time: swear the affidavit for delayed death registration and file it with the application.

To have the deceased’s name removed from a family record or to claim their dues, apply for a surviving member certificate.
