What you will need

Use the papers in this order.

  1. Before you start, collect these papers.

    Have ready:

    • ID and address proof of the elderly person, of the family member who will act for them and of the attendant
    • The name and phone number of the family doctor

  2. To hire an attendant or nanny for the elderly person, make the elder care attendant agreement. It records the duties, the pay and the hours, and both sign.

  3. The elderly person makes the medical power of attorney, naming a person to decide about their medical treatment if they can no longer decide for themselves.

  4. The elderly person also gives a family member the power to handle their affairs: make the power of attorney for an elderly parent and sign it with two witnesses.
