What you will need

Use the papers in this order.

  1. Before you start, collect these papers.

    Have ready:

    • The insurance policy and the policy number
    • Bills, hospital or repair papers, photographs and the insurer’s claim form

  2. For a loss of property by fire, make the property / fire insurance claim affidavit and send it to the insurer with your claim.

  3. For a health claim, make the health insurance claim letter and send it to the insurer or its third-party administrator.

  4. If the insurer rejects the claim, make the appeal against claim rejection and send it to the insurer’s grievance officer.
