What you will need
Use the papers in this order.
Before you start, have these ready.
Have ready:
- Your old passport, if any, and proof of your date of birth and address
- Marriage or divorce papers, if your name or spouse’s name is changing
Swear the passport affidavit in the annexure the Passport Office asks for (D, F or I) for your case.
- Passport Annexure Affidavit (Annexure D / F / I) (made below, with your kit details)
To add your spouse’s name to the passport, swear the spouse name addition affidavit (Annexure J).
- Passport Spouse Name Addition (Annexure J) (made below, with your kit details)
If the date of birth on your papers differs, swear a date of birth affidavit.
- Date of Birth Affidavit (made below, with your kit details)
If your name is spelt differently on the passport, marksheets or an IELTS report, swear a name mismatch affidavit.

If your passport or another document is lost, swear a lost document affidavit and report the loss as the office requires.
