- 4. That none of the members named above holds, or is included in, any other ration card issued anywhere in India, and that I have not concealed any such other card.
- 5. That the particulars given above and in the accompanying application are true, and I undertake to inform the Department promptly if any of them changes.
- 6. That the contents of this affidavit are true to the best of my knowledge and belief, and I am aware that obtaining a ration card, or a corrected entry on it, on a false declaration is an offence and renders the card liable to cancellation.
