The distributor's own transfer procedure follows the marketing discipline guidelines its oil marketing company issues, not a statute specific to Punjab or to Chandigarh, so what is jurisdiction-specific here is chiefly where this affidavit is notarised and on what value of stamp paper.

ਰਜਿਸਟਰੀ

ਇਹ ਹਲਫ਼ਨਾਮਾ ਵਿਤਰਕ ਕੋਲ ਉਸਦੇ ਆਪਣੇ ਰਿਕਾਰਡ ਲਈ ਦਾਖ਼ਲ ਕੀਤਾ ਜਾਂਦਾ ਹੈ ਅਤੇ ਇਹ ਅਜਿਹਾ ਸਾਧਨ ਨਹੀਂ ਹੈ ਜਿਸਨੂੰ ਪੰਜਾਬ ਵਿੱਚ ਜਾਂ ਚੰਡੀਗੜ੍ਹ ਵਿੱਚ ਰਜਿਸਟ੍ਰੇਸ਼ਨ ਐਕਟ, 1908 ਅਧੀਨ ਰਜਿਸਟਰ ਕਰਵਾਉਣ ਦੀ ਲੋੜ ਹੋਵੇ। ਤਬਾਦਲਾ ਬੇਨਤੀ ਦੀ ਵਿਤਰਕ ਦੀ ਰਸੀਦ ਸੰਭਾਲ ਕੇ ਰੱਖੋ।

I, Ravinder Kumar, son of Sh. Mohinder Lal, resident of House No. 41, Model Town, Jalandhar, Punjab 144003, holding Aadhaar 5555 6666 7777, ordinarily resident within the State of Punjab (the "Deponent" or "I"), do hereby solemnly affirm and declare as follows:

1. THE CONNECTION

1.1 This affidavit concerns a domestic LPG cylinder connection bearing consumer number IG-0441-778899 with Jalandhar Indane Gas Service, Model Town, Jalandhar, standing at present in the name of Late Sh. Mohinder Lal, for the premises at House No. 41, Model Town, Jalandhar, Punjab 144003.

2. REASON FOR TRANSFER

2.1 I seek transfer of this connection into my name because of the death of the existing consumer, this affidavit being sworn by a legal heir.

2.2 Particulars of the death: Died 2 January 2026 at Civil Hospital, Jalandhar; death certificate No. J/2026/00456 dated 15 January 2026

2.4 My relationship to Late Sh. Mohinder Lal is: Son and legal heir.

3. DUES

3.1 There are no outstanding dues against this connection

3.2 I undertake to clear any dues found outstanding against this connection up to the date of transfer, and I indemnify Jalandhar Indane Gas Service, Model Town, Jalandhar against any claim, loss or demand arising from a due, a defect in my title to succeed to or acquire the connection, or a statement in this affidavit that later proves incorrect.

4. SAFETY UNDERTAKING

4.1 I undertake to use the connection only at the premises stated above, for domestic purposes, and to comply with the distributor's safety and inspection requirements. I confirm that the gas stove, regulator and suction hose at the premises have been checked and are safe.

5. DECLARATION

5.1 Every statement made above is true to my own knowledge and belief, and I have concealed nothing material to this transfer. I am aware that furnishing false information to obtain a gas connection is an offence, and that Jalandhar Indane Gas Service, Model Town, Jalandhar may refuse or cancel the transfer if any statement made here is found false.
