When you need it

A plain sworn statement of your own facts, for an office, bank, school or other institution in Punjab or Chandigarh that asks you to give one — for example your address, a fact about your family, or a straightforward declaration that something is true. You write up to five statements in your own words; this template supplies the heading, the oath wording, the verification and the notary's attestation block.

If the affidavit is for one particular purpose, use the dedicated affidavit instead — the Name Change Affidavit (Adult), the Income Affidavit, the Lost Document Affidavit (Passport / Share Certificate), or the Affidavit of One and the Same Person; each is written to match the proforma an office expects for it. If it is to support a case or application pending before a court or tribunal, use the General Court Affidavit.

What follows is written for Punjab and Chandigarh. It states the position there rather than describing how it varies, so the amounts below are the ones that apply — worth confirming at the sub-registrar’s office, because rates change. If the property is anywhere else in India, none of this is yours.

Stamp duty

Punjab: Article 4, Schedule 1-A to the Indian Stamp Act, 1899 as applicable to Punjab, non-judicial stamp paper, commonly Rs 10, e-stamped via SHCIL (shcilestamp.com) or bought from a licensed vendor. Chandigarh: Article 4, Schedule I to the Indian Stamp Act, 1899 as extended to the Union Territory, the same nominal denomination, from SHCIL or the Treasury, Sector 17. Some banks, schools and employers accept an affidavit on plain paper for an internal file; confirm what the recipient wants before buying stamp paper. An affidavit filed in a pending court case is different: it carries the court's own court-fee stamp instead — use the General Court Affidavit for that.

Registration

Not registrable in either jurisdiction — an affidavit creates no right in property or otherwise, so Section 17 of the Registration Act, 1908 does not reach it. It is handed to the office, bank or school that asked for it, which keeps it on its file; get a separate original sworn for each recipient.

Notarisation

Required — an unsworn affidavit is only a signed statement and carries no weight as one. Swear it before a Notary Public under the Notaries Act, 1952, an Oath Commissioner or an Executive Magistrate. The deponent appears in person with photo identification and signs in front of the attesting officer, who writes the register serial number, the date and the seal on the affidavit.

Witnesses

None required by law — the deponent's signature and the attesting officer's jurat are the whole formality. Some offices ask for a photocopy of the deponent's photo identification to be attached; check with the recipient before the affidavit is sworn.
