AFFIDAVIT FOR CHANGE OF NAME OF A MINOR

I, Rohini Sharma, father of the minor named below, resident of House No. 2214, Sector 15, Panchkula 134113, do hereby solemnly affirm and declare as under:

1. That I am the deponent above named and am competent to swear this affidavit as the minor's natural guardian under section 6 of the Hindu Minority and Guardianship Act, 1956.

2. That the minor was born on 1 April 2026 and was recorded under the name Aarav.

Birth Certificate No. PB/2016/0417293.
