When you need it

For an application to the Food, Civil Supplies and Consumer Affairs Department in Punjab, or the Food and Supplies Department in Chandigarh, for a new ration card, or to add a family member — on birth or on marriage into the family — or remove one — on death, on marriage out, or on the member permanently shifting to another household — from an existing card. This is the sworn declaration those departments ask an applicant to annex, confirming the family composition stated in the application and, importantly, that no member named is already covered under another ration card anywhere in India — the fact the National Food Security Act's one-card-per-family rule and Aadhaar seeding are both designed to catch.

Filed through the Punjab Food Supply portal or a Suwidha Kendra, or Chandigarh's Sampark Kendra or e-District portal, alongside the department's own application form.

What follows is written for Punjab and Chandigarh. It states the position there rather than describing how it varies, so the amounts below are the ones that apply — worth confirming at the sub-registrar’s office, because rates change. If the property is anywhere else in India, none of this is yours.

Stamp duty

Punjab: Article 4, Schedule 1-A to the Indian Stamp Act, 1899 as applicable to Punjab, nominal non-judicial stamp paper, commonly Rs 10, e-stamped via SHCIL (shcilestamp.com) or bought from a licensed vendor. Chandigarh: Article 4, Schedule I, Indian Stamp Act 1899 as extended to the Union Territory, the same nominal denomination, from SHCIL or the Treasury, Sector 17.

Registration

Not a registrable instrument. Annexed to the ration card application filed with the Food, Civil Supplies and Consumer Affairs Department (Punjab) or the Food and Supplies Department (Chandigarh), which verifies the family composition against Aadhaar seeding and the existing National Food Security Act database before acting on it. There is no Sub-Registrar step.

Notarisation

Sworn before a Notary Public or an Executive Magistrate/Oath Commissioner. Where filed on the spot at a Suwidha Kendra or Sampark Kendra counter, that counter's own attesting official may accept the affidavit in their presence in place of prior notarisation — confirm with that counter first.

Witnesses

None required by law for the affidavit itself.
