कब ज़रूरत पड़ती है

You are applying for the Old Age Pension or the Widow Pension (also called Financial Assistance to a Destitute Woman) administered in Punjab by the Department of Social Security, Women and Child Development through the Sewa Kendra network, or in Chandigarh by the Social Welfare Department, Chandigarh Administration — and the application form asks for a sworn affidavit confirming your age, marital status, family income, residence, and that you are not already drawing a conflicting pension. Both schemes are welfare benefits, part-funded under the National Social Assistance Programme and topped up by the State or the Administration, sanctioned on age (or widowhood) and income criteria that are revised from time to time and checked again at renewal.

रजिस्ट्री

यह शपथ-पत्र अचल संपत्ति को प्रभावित नहीं करता और यह पंजाब या चंडीगढ़ में रजिस्ट्रीकरण अधिनियम, 1908 द्वारा पंजीकृत किए जाने की अपेक्षा वाला लिखत नहीं है। यह पंजीकृत नहीं किया जाता; मूल प्रति स्वीकृति देने वाले कार्यालय द्वारा रखी गई पेंशन फ़ाइल में जाती है, और शपथकर्ता को एक हस्ताक्षरित और अभिप्रमाणित फ़ोटोकॉपी रखनी चाहिए।

नोटरी

पंजाब या संघ राज्यक्षेत्र चंडीगढ़ के लिए नियुक्त एक नोटरी पब्लिक या शपथ आयुक्त के समक्ष शपथ ली या प्रतिज्ञान किया गया, जो शपथकर्ता की पहचान सत्यापित करता है, शपथ या प्रतिज्ञान दिलाता है, और शपथ-पत्र पर हस्ताक्षर, मुहर और तारीख़ अंकित करता है। जहाँ दाख़िल करने वाला कार्यालय इस विशेष लाभ के लिए एक सादी स्व-घोषणा स्वीकार करता है, जैसा ऊपर बताया गया, वहाँ यह चरण छोड़ दिया जाता है और शपथकर्ता सीधे कार्यालय के अपने अधिकारी के समक्ष हस्ताक्षर करता है — नियुक्ति बुक करने से पहले पुष्टि करें कि दाख़िल करने वाला कार्यालय कौन-सा मार्ग चाहता है।

गवाह

एक शपथ-पत्र नोटरी या शपथ आयुक्त के अपने अभिप्रमाणन द्वारा प्रमाणित होता है और दोनों क्षेत्राधिकारों में किसी अलग गवाह की आवश्यकता नहीं है। जहाँ दाख़िल करने वाले कार्यालय के अपने आवेदन फ़ॉर्म में अतिरिक्त रूप से एक या दो पहचान गवाहों की माँग हो — जैसा कुछ सेवा केंद्र और संपर्क केंद्र काउंटर क़ानून के बजाय स्थानीय व्यवहार के मामले के रूप में करते हैं — उस कार्यालय के फ़ॉर्म का पालन करें।

I, Gurdial Kaur, widow of Late Sh. Baldev Singh, aged about 67 years (date of birth 1 April 2026), resident of House No. 214, Bagga Mohalla, Batala, Punjab 143505, holding 4444 5555 6666, ordinarily resident within the State of Punjab (the "Deponent" or "I"), do hereby solemnly affirm and declare as follows:

1. SCHEME APPLIED FOR

1.1 I am applying for Old Age Pension, and I make this affidavit in support of that application for filing before Sewa Kendra, Batala, District Gurdaspur.

2. RESIDENCE AND ELIGIBILITY

2.1 I have been ordinarily resident within the State of Punjab for 30 years, and I continue to be ordinarily resident there as on the date of this affidavit.

2.2 My total annual family income, from all sources, is ₹48,000 (Rupees Forty Eight Thousand only).

2.3 I do not receive any other pension or financial assistance from the Central or State Government

3. BANK ACCOUNT FOR DIRECT BENEFIT TRANSFER

3.1 The pension, if sanctioned, may be credited to the following account, which stands in my own name: Gurdial Kaur, A/c 30123456789, State Bank of India, Batala Branch, IFSC SBIN0001234

4. DECLARATION

4.1 Every statement made above is true to my own knowledge and belief, and I have concealed nothing material to my eligibility for Old Age Pension.

4.2 I am aware that this is a means-tested welfare benefit, that its continuance depends on the facts stated above remaining true, that I must report in writing to Sewa Kendra, Batala, District Gurdaspur any change in my income, any remarriage where this is a widow's pension, any other pension I come to receive, or any change of residence, that a benefit drawn on a false statement is recoverable, and that making a false declaration in a sworn affidavit is an offence.

VERIFICATION

Verified at Batala on 2 April 2026 that the contents of paragraphs 1 to 4 above are true and correct to my own knowledge, that no part of it is false, and that nothing material has been concealed.
