A sworn affidavit, combined with an indemnity bond, for a marksheet, certificate, degree or similar academic document that has been lost — used to support an application to the issuing Board, University or School for a duplicate. Records the loss, the search made for the original, whether a police report or online lost-property complaint was lodged, and whether a public notice was published, and indemnifies the institution against the consequences of issuing a duplicate.

Two instruments, each separately chargeable, so Part A and Part B are engrossed as separate stamped papers rather than one — Section 5 of the Indian Stamp Act, 1899 taxes an instrument covering distinct matters at the aggregate of the duties applicable to each, and keeping them apart avoids any dispute over which single rate applies. Part A, the affidavit, is chargeable under Article 4, Schedule I-A (Punjab) — a nominal fixed duty authorities commonly print as Rs 25, though no official table carries a specific affidavit entry — confirm at the counter; Chandigarh applies its own Schedule I figure. Part B, the indemnity bond, is chargeable under Article 34 of the applicable Schedule — a fixed duty higher than the affidavit's, revised from time to time by notification; confirm the current figure at the stamp counter or the e-stamp portal before buying the stamp paper. Buy both in the holder's own name, and check whether the receiving institution prescribes its own stamp value for either part, which some Boards and Universities do.

रजिस्ट्री

Not registrable — neither Part A nor Part B passes any interest in immovable property, so Section 17 of the Registration Act, 1908 does not reach either. Some institutions describe the indemnity bond they want as 'registered', meaning only notarised; confirm which is meant before assuming a Sub-Registrar is involved.

नोटरी

Both parts are sworn or executed before a Notary Public, an Oath Commissioner, or an Executive Magistrate, with the holder appearing in person with photo identification. Execute both on the same day before the same officer, even though they are separate stamped instruments, so the dates match.

गवाह

Part A, the affidavit, needs none by law. Part B, the indemnity bond, has no statutory requirement either, but two attesting witnesses — adults, not the holder's dependants — is near-universal institutional practice; provide them unless the receiving institution's own format says otherwise.

AFFIDAVIT AND INDEMNITY BOND FOR A LOST ACADEMIC CERTIFICATE

PART A — AFFIDAVIT OF LOSS

I, Jaspreet Kaur, D/o Sh. Balbir Singh, resident of House No. 65, Model Town, Ludhiana, contact number 98765 43210, do hereby solemnly affirm and declare as under:

1. That I was issued a mark sheet / statement of marks, namely X, bearing number PSEB/10/2019/558214, by Punjab School Education Board, in the year 2019.

2. That the said certificate has been lost under the following circumstances: the certificate was kept in a file of personal documents at the holder's residence, which was found missing while preparing documents for a university application in July 2026; it is believed to have been misplaced during a change of residence in March 2026.

3. That the certificate was last seen in my possession on or about 1 April 2026.

4. That despite the following efforts, the certificate could not be traced: a thorough search was made at the holder's residence and former residence, and enquiries were made with family members, without success

5. That a First Information Report / General Diary entry has been lodged with the police, particulars of which are given below

Particulars: Daily Diary Report No. 412 dated 20 July 2026, Police Station Division No. 4, Ludhiana

6. That a public notice regarding the loss has been published in a newspaper, particulars of which are given below

Particulars: Published in the Daily Ajit, Ludhiana edition, dated 22 July 2026

7. That the said certificate has not been pledged, deposited, mortgaged or transferred to any person or institution, and remains untraced despite the efforts stated above.

8. That this affidavit is sworn in support of an application for issuance of a duplicate certificate in place of the one lost.

9. That the contents of this affidavit are true and correct to my knowledge and belief, that no part of it is false and that nothing material has been concealed therefrom.

PART B — INDEMNITY BOND

I, Jaspreet Kaur, D/o Sh. Balbir Singh, resident of House No. 65, Model Town, Ludhiana, in consideration of Punjab School Education Board agreeing to issue a duplicate of a mark sheet / statement of marks bearing number PSEB/10/2019/558214 in place of the original lost as stated in Part A above, do hereby undertake and bind myself as follows:

10. That I shall indemnify and keep indemnified Punjab School Education Board against any loss, claim, demand or liability that may arise out of, or in connection with, the issuance of the duplicate certificate, including any loss, claim or liability arising from misuse of the original by any person into whose hands it may come.
