You are about to engage, or have just engaged, domestic help, a cook, a driver, a security guard, a gardener or similar staff in Punjab or Chandigarh, and want their antecedents checked with the police before you hand them keys, cash, a vehicle or access to your home or premises unsupervised. This is the application that starts that check: your own particulars as the employer, the employee's identity and address particulars, and the employee's own consent to being verified.

Punjab Police runs this as a citizen service through Sanjh Kendras and its online citizen portal, alongside the same tenant-verification service; Chandigarh Police runs the equivalent through its local police stations and citizen portal. Neither police force publishes a standalone statute compelling a private household to get a domestic worker verified — the practice rests on police standing instructions and advisories, not a Punjab or Chandigarh Act naming domestic employment specifically — but both forces accept and act on this application, and many housing societies and RWAs now ask for a copy before letting staff onto the premises. If the guard is supplied by a licensed private security agency rather than engaged by you directly, the agency itself is separately required to verify its guards' antecedents before deployment, under s.9 of the Private Security Agencies (Regulation) Act, 2005 — see the Security Agency Agreement for that route. Use this form yourself where you are the direct employer, or where you want your own independent verification in addition to the agency's. Not for a tenant or paying guest — use the Tenant and Paying Guest Police Verification Report for that. This is an application addressed to a Station House Officer, not an instrument the Indian Stamp Act taxes — it transfers no right, creates no interest and is not a conveyance, agreement for consideration or power of attorney. No stamp paper or e-stamp is needed for it in either Punjab or Chandigarh, and a police station that asks you to buy stamp paper for a verification application is asking for something this form does not require.

Not applicable. This form is never presented to a Sub-Registrar and the Registration Act, 1908 has nothing to say about it — it does not touch immovable property and creates no registrable interest. What "registration" means here is entirely different: Punjab Police's own citizen portal and Sanjh Kendra network, and Chandigarh Police's citizen portal, both let you submit and track this application online, and the local police station keeps its own record of it. Submit it at, or address it to, the police station having jurisdiction over your residence or business premises in Punjab, or over your premises in the Union Territory of Chandigarh; the station typically forwards the employee's permanent-address particulars to the police station of that area for a local report, which is why the permanent address must be complete and accurate.

Where the employer is a company, firm, RWA or society, attach a copy of the resolution or letter authorising the person who signs. Where the employee's own signature cannot practically be taken in person before submission, take it as soon as the employee is available and before relying on any verification obtained without it.
