कब ज़रूरत पड़ती है

Your name or date of birth is wrong on your Aadhaar card — a spelling error made at enrolment, a name that changed on marriage, or a date of birth recorded from an estimate rather than a document — and the Unique Identification Authority of India's own correction process, through the Self Service Update Portal, the myAadhaar portal, or an Aadhaar Seva Kendra, asks you to submit a document it accepts as proof, together with a signed declaration of what the correct particulars are and how the error arose. Aadhaar enrolment and update is governed by the Aadhaar (Targeted Delivery of Financial and Other Subsidies, Benefits and Services) Act, 2016 and the regulations made under it, which apply identically wherever in India the correction is made; what differs between Punjab and Chandigarh is only where this declaration is notarised, if the update centre you use asks for a notarised copy rather than a plain signed one.

यह घोषणा भारतीय विशिष्ट पहचान प्राधिकरण को उसके अपने अभिलेख के लिए प्रस्तुत की जाती है और यह ऐसा लिखत नहीं है जिसे रजिस्ट्रीकरण अधिनियम, 1908 द्वारा पंजाब या चंडीगढ़ में पंजीकृत किया जाना आवश्यक हो। पोर्टल या केंद्र द्वारा जारी पावती और अपडेट रिक्वेस्ट नंबर सुरक्षित रखें — यह घोषणा नहीं बल्कि वह नंबर है जो सुधार को ट्रैक करता है।

नोटरी

सेल्फ़ सर्विस अपडेट पोर्टल और myAadhaar पोर्टल इस घोषणा को एक सादे हस्ताक्षर के साथ, सहायक दस्तावेज़ के साथ अपलोड की गई, स्वीकार करते हैं — ऑनलाइन कोई नोटरीकरण नहीं माँगा जाता। एक आधार सेवा केंद्र या अपडेट केंद्र, या नामांकन एजेंसी के रूप में कार्य करने वाला कोई बैंक या डाकघर, इसे पंजाब या संघ राज्यक्षेत्र चंडीगढ़ के लिए नियुक्त नोटरी पब्लिक या शपथ आयुक्त के समक्ष नोटरीकृत कराने के लिए कह सकता है; नियुक्ति से पहले संबंधित केंद्र से पूछें।

गवाह

AADHAAR NAME / DATE OF BIRTH CORRECTION DECLARATION

I, Harpreet Singh Dhillon, son of Sh. Amarjit Singh Dhillon, resident of House No. 88, Urban Estate Phase II, Patiala, Punjab 147002, holding Aadhaar number XXXX XXXX 1234, ordinarily resident within the State of Punjab (the "Deponent" or "I"), do hereby solemnly declare as follows:

1. THE ERROR

1.1 Correction of the name only is required in my Aadhaar record.

1.2 The name currently printed on my Aadhaar card is "Harpreet Singh Dhilon", which is incorrect. My correct name, as given above, is "Harpreet Singh Dhillon".

1.4 This error arose as follows: The enrolment operator mistyped the spelling of my surname at the time of enrolment in 2013.

2. SUPPORTING DOCUMENT

2.1 In support of the correct particulars declared above, I rely on and am submitting the following document, of the kind the Unique Identification Authority of India accepts for this purpose: Passport No. M1234567, issued by the Regional Passport Office, Chandigarh, dated 4 June 2019

3. HOW THIS DECLARATION IS BEING FILED

3.1 I am submitting this declaration, together with the supporting document referred to above, through the Aadhaar Self Service Update Portal or the myAadhaar portal, uploading a scanned copy of the supporting document.

4. DECLARATION

4.1 Every statement made above is true to my own knowledge and belief. I have not sought this correction to assume a false identity or to defeat any other law, and I have not concealed any material fact.

4.2 I am aware that the Unique Identification Authority of India may verify this declaration and the supporting document against its own records before accepting the correction, may ask for an additional or a different document, and that furnishing false information for inclusion in the Aadhaar database is an offence under the Aadhaar (Targeted Delivery of Financial and Other Subsidies, Benefits and Services) Act, 2016.
