Confirm the currently accepted list before the appointment, and sign only in the attesting officer's presence.

I, Ritu Sood, son/daughter of Mohan Lal Sood, born on 1 April 2026, resident of House No. 918, Ranjit Avenue, Amritsar 143001, do hereby solemnly affirm and declare as under:

1. That I am the deponent above named and am competent to swear this affidavit.

2. That I hold passport number P7654321, issued 11 May 2021 at Amritsar.

3. That I was married to Anmol Sood on 2 April 2026 at Amritsar.

4. That The marriage has been registered, as recorded below.

Registration particulars: Marriage Certificate No. ASR/2021/003142 issued by the Local Registrar of Marriages, Amritsar.

5. That my spouse, Anmol Sood, is presently residing at House No. 918, Ranjit Avenue, Amritsar 143001. My spouse's date of birth is 3 April 2026. My spouse's passport number is X.

6. That I request the name of my spouse, Anmol Sood, be added to and incorporated in my passport records accordingly.
