For an applicant seeking a Backward Class (BC) or Scheduled Caste (SC) certificate from the Tehsildar/SDM in Punjab or the Deputy Commissioner's office in Chandigarh, where the application asks for a sworn affidavit supporting the claimed caste, native place and — for a Backward Class claim only — that the family is outside the creamy layer. A Scheduled Caste claim carries no creamy layer exclusion: the creamy layer test applies only to Backward Classes/OBCs, following Indra Sawhney v. Union of India.

ਇੱਕ ਨੋਟਰੀ ਪਬਲਿਕ (ਨੋਟਰੀ ਐਕਟ, 1952) ਜਾਂ ਇੱਕ ਕਾਰਜਕਾਰੀ ਮੈਜਿਸਟਰੇਟ/ਸਹੁੰ ਕਮਿਸ਼ਨਰ ਸਾਹਮਣੇ ਸਹੁੰ ਚੁੱਕ ਕੇ। ਜਿੱਥੇ ਸੁਵਿਧਾ ਕੇਂਦਰ ਜਾਂ ਸੰਪਰਕ ਕਾਊਂਟਰ ਉੱਤੇ ਮੌਕੇ ਉੱਤੇ ਸਹੁੰ ਚੁੱਕੀ ਜਾਂਦੀ ਹੈ, ਉੱਥੇ ਉਸ ਕਾਊਂਟਰ ਦਾ ਆਪਣਾ ਅਭਿਪ੍ਰਮਾਣਕ ਅਧਿਕਾਰੀ ਪਹਿਲਾਂ ਦੇ ਨੋਟਰੀਕਰਨ ਦੀ ਥਾਂ ਆਪਣੀ ਹਾਜ਼ਰੀ ਵਿੱਚ ਹਲਫ਼ਨਾਮਾ ਸਵੀਕਾਰ ਕਰ ਸਕਦਾ ਹੈ — ਪਹਿਲਾਂ ਸਬੰਧਤ ਕਾਊਂਟਰ ਤੋਂ ਪੁਸ਼ਟੀ ਕਰੋ।

  1. 2. That my family's native village, Tehsil and District is Village Ghanaur, Tehsil and District Patiala, being the place against which this claim falls to be verified.

Existing family certificate: Caste certificate No. 4521/SC dated 12 May 1998 issued by Tehsildar, Patiala, relied on in support of this claim.

  1. 3. That neither I nor any member of my family has converted to a religion outside those recognised for this caste to remain within the Scheduled Caste (SC) list, and that this claim is made truthfully and not to secure a benefit I am not entitled to.
