What follows is the position for this kind of document. The amount is set by the state the property is in and it changes, so the figure for your own state is worth confirming at the sub-registrar’s office before you pay anything.

Stamp duty

A fixed, nominal duty under Article 4, Schedule I — NOT ad valorem; it doesn't depend on the value of any property or claim. In Punjab and Chandigarh it's Rs 10; other states range roughly Rs 10-100 (Maharashtra Rs 100, Karnataka/Tamil Nadu Rs 20) — verify the current figure with the stamp vendor or e-stamp portal, since states revise these by notification. Buy a higher denomination if unsure — that's harmless. Buying too little is a real risk: an under-stamped affidavit can be impounded (s.33) and released only on paying the deficit plus a penalty up to 10x (s.35/39). E-stamp certificates (SHCIL or the state's own system) are widely available; physical stamp paper works everywhere too. One exemption: an affidavit made for filing in a court is exempt from Article 4 duty — it instead carries a court-fee stamp and is usually sworn before a court-attached Oath Commissioner, not a notary.

Registration

Not registrable — not in section 17's compulsory list, and no reason to use section 18 either; skip the Sub-Registrar. It's filed as an ANNEXURE instead — in a mutation file, alongside a deed, or with a bank/EPFO/LIC/society request — and stays there. Get a separate ORIGINAL sworn for each office you file with; each needs its own stamp paper, since photocopies are usually refused.

Notarisation

Yes — the whole point; unsworn, it's just a letter. Swear it before a Notary Public, an Oath Commissioner, or an Executive/Judicial Magistrate, in person, with your ID and documents showing each name version. Three things must be on the finished affidavit, or it's commonly rejected: the notary's seal/signature; the register serial number and date written on the page (checked routinely); and the separate notarial stamp. A notary typically charges Rs 100-300 all-in. No statutory expiry, but banks, EPFO and the passport office often refuse one older than 3-6 months.

Witnesses

None required by law — the deponent's signature plus notary attestation is the whole formality; don't add witnesses out of habit. Some offices ask extra: an identifying person who vouches for you before the notary; occasionally a gazetted officer's counter-attestation (EPFO); or a photo cross-signed and notary-stamped (banks, LIC, passport). Sign the same way on every page and initial each — most notaries won't attest an uninitialled multi-page affidavit.

I, Sunita Sharma, S/o Ram Kishan Sharma, aged about 41 years, by occupation Service, resident of House No. 214, Sector 12, Sonipat, Haryana - 131001, holding Aadhaar bearing No. XXXX XXXX 4821, do hereby solemnly affirm and declare as under:

1. That I am the deponent named above. I am a citizen of India, I am of sound mind and competent to swear this affidavit, and the facts stated below are true to my own personal knowledge.

2. That this affidavit concerns my own name and identity — save that, instead, it concerns the name and identity of Aarav Sharma, a minor, my son, born on 1 April 2026, of whom I am the natural guardian, and on whose behalf I make this affidavit from facts within my personal knowledge.

3. That I am making this affidavit for the following purpose: To have the mutation (dakhil kharij) of Khasra No. 118/2, Village Kharkhoda, entered in my name, where the sale deed records me as Sunita Sharma but the jamabandi and my Aadhaar record me as Sunita Devi. This affidavit is being submitted to the office of the Tehsildar, Office of the Tehsildar, Kharkhoda, District Sonipat. It relates to Mutation Application No. 1476 / Khasra No. 118/2, Khewat No. 42, Village Kharkhoda.

4. That the name of the person described in paragraph 2 above has been recorded differently in different documents, in the following versions:

(a) "Sunita Sharma" — as appearing in: Aadhaar card, PAN card, Sale Deed dated 12.03.2021 registered as Vasika No. 3341.

(b) "Sunita Devi" — as appearing in: Jamabandi for the year 2018-19, Village Kharkhoda; ration card; electricity bill for Consumer No. 4402118.

(c) "S. Sharma" — as appearing in: School Leaving Certificate issued by Govt. Sr. Sec. School, Kharkhoda, dated 30.04.1999.

(d) "Sunita" — as appearing in: Bank passbook, Account No. 3092xxxx1104, Punjab National Bank, Kharkhoda branch.

Further, the name of a relative of mine, namely my father, has likewise been recorded differently in the same set of documents, namely as "Ram Kishan Sharma" and as "Ramkishan", appearing in: 'Ram Kishan Sharma' in my Aadhaar and PAN; 'Ramkishan' in the jamabandi and in my School Leaving Certificate. Every such version refers to one and the same person, that is to say my said relative, whose correct and complete name is "Ram Kishan Sharma".

5. That all the versions of the name set out in paragraph 4 above refer to one and the same person, that is to say the person described in paragraph 2 above, and to no other person whatsoever. There is no second individual answering to any of those versions in the documents referred to.

What this document is for

This is used when one and the same person's name is written differently across their own papers — a maiden name against a married name, an initial spelled out in one document but not another, a spelling that drifted when a jamabandi written in Urdu or a regional script was transliterated — and an office now needs to be satisfied that every version refers to one person and nobody else.

It exists because a name mismatch between two documents — say, the spelling on a sale deed against the spelling in the jamabandi, or an old school record against a current Aadhaar — can quietly stall a mutation, a bank KYC check, an EPFO or LIC claim, or a passport or RTO application until somebody formally reconciles the two. This affidavit is that formal reconciliation, sworn once and then filed wherever the mismatch is being raised.

This affidavit does not change your name — adopting a genuinely new name needs a deed of change of name, usually followed by Gazette publication, which is a different process entirely. No — Not registrable — an affidavit does not appear on the Registration Act, 1908's compulsory list, and there is no benefit to registering one even where the option exists elsewhere. It is filed as an ANNEXURE wherever it is actually needed — inside a mutation file, alongside a deed, or with a bank, EPFO or society request — rather than lodged anywhere on its own.

Notarisation is the whole point of this document — an unsworn version is just a signed letter with no evidentiary weight. No witness is required by law, though some receiving offices separately want a person who can identify you before the notary, or a photograph cross-signed and notary-stamped.

How much stamp duty does an affidavit of one and the same person need in Punjab or Chandigarh?

Does this affidavit change my name?

Will this affidavit fix the spelling of my name on my Aadhaar or PAN?

Not directly.
