You are applying to a Municipal Corporation or Municipal Council for a trade licence to run a trade the municipal law treats as one requiring a licence — commonly the "dangerous and offensive" trades scheduled under the applicable municipal Act, such as an eatery or dhaba using fire, a business storing or selling chemicals, timber, scrap, or similar goods, or another trade the local byelaws specify — and the municipal office wants a sworn affidavit from the applicant along with the application form. In Punjab this is issued under the Punjab Municipal Corporation Act, 1976 in a Corporation area, or the Punjab Municipal Act, 1911 in a Municipal Council or Nagar Panchayat area; in Chandigarh, under the Punjab Municipal Corporation Act, 1976 as extended to and applied in the Union Territory, administered by the Municipal Corporation, Chandigarh.

A Rs 10 non-judicial stamp paper is commonly used for an affidavit of this kind in both Punjab and Chandigarh; confirm the value currently accepted with the notary or Oath Commissioner before the affidavit is engrossed.

रजिस्ट्री

Not registrable under the Registration Act, 1908. This affidavit supports a trade licence application made to the Municipal Corporation or Municipal Council for the area, a separate process from registration of a document.

नोटरी

Required. This is an affidavit and must be sworn before a Notary Public appointed under the Notaries Act, 1952, or before an Oath Commissioner or Executive Magistrate authorised to administer oaths.

AFFIDAVIT

I, Sandeep Singh Gill, S. Jarnail Singh Gill, aged 41 years, resident of House No. 512, Urban Estate, Phase II, Patiala 147002, do hereby solemnly affirm and declare as follows.

1. I am the sole proprietor of the trade known as "Gill Dhaba", carried on at Booth No. 7, Grain Market Road, Patiala 147001 (the "Premises"), and I am competent to swear this affidavit from my own personal knowledge.

2. The Premises are situated in the State of Punjab.

3. The trade carried on is: A dhaba serving cooked food, using LPG-fired cooking equipment on the premises.

4. the Deponent, or the entity on whose behalf the Deponent swears this affidavit, is the owner of the premises

5. The following clearance has been obtained for this trade: Fire safety NOC No. FSO/PTL/2026/118 dated 4 August 2026 from the Fire Officer, Patiala.

6. This affidavit is sworn in support of a fresh trade licence application before Municipal Corporation, Patiala.

7. The existing trade licence number is TL/PTL/2023/2214.

8. I undertake to carry on the trade strictly in accordance with the conditions of the licence and the applicable municipal Act and byelaws, to renew the licence when it falls due, to obtain and keep current every fire safety, pollution or other clearance the trade requires, and to inform Municipal Corporation, Patiala promptly of any change in the trade or the Premises.

9. I further declare that no trade licence of mine has been revoked or cancelled for violation of its conditions, that no proceeding for such cancellation is pending, and that the particulars given above are within my own knowledge and are true and correct, and that nothing material has been concealed.
