LOST DOCUMENT AFFIDAVIT
Executed at Chandigarh on 2 April 2026.
I, Rajwinder Kaur, daughter of Sh. Sukhdev Singh, resident of House No. 505, Sector 21-C, Chandigarh 160022, holding Aadhaar 3333 4444 5555, ordinarily resident within the State of Punjab (the "Deponent" or "I"), do hereby solemnly affirm and declare as follows:
- 1. THE DOCUMENT LOST
1.1 This affidavit concerns the loss of a passport, addressed to Regional Passport Office, Chandigarh.
1.2 Passport particulars: Passport No. N1234567, issued at Chandigarh on 4 March 2019, valid until 3 March 2029.
- 2. CIRCUMSTANCES OF LOSS
2.1 The document was lost on or about 1 April 2026. Misplaced while shifting residence; not found despite a thorough search of the old premises.
2.2 Search made before concluding it is lost: Searched the residence, the office and all bags used while travelling; enquired with the courier used for the last delivery.
- 3. REPORT AND NOTICE
3.1 A police report (FIR) has been lodged, particulars of which are given below.
Particulars: e-FIR No. 4021/2026 dated 9 September 2026, Police Station Sector 17, Chandigarh.
3.2 A public notice of the loss has been published, particulars of which are given below.
Particulars: Published in the Tribune, Chandigarh edition, dated 12 September 2026.
- 4. UNDERTAKING AND INDEMNITY
4.1 I undertake that if the original document is subsequently found or comes into my possession or control, I shall immediately surrender it to Regional Passport Office, Chandigarh — or, if it is the passport, deposit it with the passport issuing authority — and shall not use it further.
4.2 I indemnify Regional Passport Office, Chandigarh against any loss, claim, demand or liability arising from the issue of a duplicate or replacement in reliance on this affidavit, or from any statement made here that later proves incorrect.
- 5. DECLARATION
5.1 Every statement made above is true to my own knowledge and belief, and I have concealed nothing material to this affidavit. I am aware that making a false declaration in an affidavit is an offence.

Verified at Chandigarh on 2 April 2026 that the contents of paragraphs 1 to 5 above are true and correct to my own knowledge, that no part of it is false, and that nothing material has been concealed.
